How come the word 'form'can be both in the singular and in plural when used here?

It depends on whether the noun relates to each one separately or all together:

In the form of rain, (the form of) snow, and (the form of? ) so on.
In the forms of (rain, snow and so on).

Monday and Tuesday were fine days.
Monday and Tuesday it was a fine day
.
