Correct spacing on abbreviations (Ph.D., U.S., etc.)

Does anyone know the standard rule for spacing between multi-letter abbreviations? In other words is the correct set of abbreviations:

Ph.D., B.A., U.S.A.

or is it

Ph. D., B. A., U. S. A.?

I feel like I've seen each of these printed both ways.
