Correct my mistakes please. Thank you!

Please help me to check my letter, thank you!

To whom it may concern:

Please allow me to apologize for presenting an invalid/outdated medical certificate for my sick leave.

On March 30, I found myself in circumstance that were beyond my control and had no choice but to take time off from work and rest. I was not feeling well that day but wasn't able to see a doctor immediately since it's Good Friday that day so there was no open clinic nearby. Two days after when I felt a little better so I decided to go to clinic for check up since there are clinics that were already open. The doctor checked me up and made a diagnosis for me. That was the only time that I got a Medical Certificate from him.

I agree that the mistake was mine, as I was not see a doctor immediately.

I hope no disciplinary action will be taken against me. I assure you that this will not happen again.

Attached herewith is the Medical Certificate as a proof of my sickness. I hope this is sufficient to explain my error. I would be grateful if you could reconsider the decision of the disciplinary actions against me.
