It is correct to say, “I hardly speak when I am at school or at work.”
However, when at a hospital, the definite or indefinite article is required.
“I hardly speak when I am at the hospital.”
However, with the preposition ‘in’ you may say, “I hardly speak when I am in hospital.” (Meaning, as a patient.)
