Coordinate relative clauses: errors?

Hello, guys!

I have two questions related to strange things I have read in my English book about relative clauses.

1) When the book explains what coordinate relative clauses are, it gives this example: "The campaign wasn't successful which was very disappointing for everyone." Shouldn't there be a comma before "which"? It sounds like this sentence requires a pause on that spot, and I believe I have always seen this sort of sentence with a comma before "which". Am I right?

2) On one exercise, I am required to correct the mistake(s) in each sentence. However, I can't find any mistake in the following one: "I didn't understand why, which made me give up." Why isn't this sentence right? It makes sense to me: "the fact that I didn't understand why" made me give up. What is wrong here?

I've just found this post, all these years after, and I completely agree with the suggestion of throwing the book away ??. I can't tell if the coursebook is the same, but the one we've been using for 11th grade EFL has exactly the same exercises, and it's full of this kind of mistakes! As a teacher, I try to make sure the students aren't mislead, but sometimes it's very frustrating! Shame on t
