Confused about how to use 'had'

Does 'had' always refer to the past tense?

Is it ok for me to just say "I told her not to come."

If I put in had: "I had told her not to come," is there a difference?

Exactly Philip.

When we use "had" is because we are talking about an action that happend before another.
