Conditional--ungrammatical sentence

"I was told if I was not able to open my mouth to 50 cm in one week, therapy be necessary because it could become TMJ."

1) Is this a conditional clause functioning as a noun clause (within a main clause)?

2) Also, shouldn't it be were not was, since it is second conditional (past subjunctive)?

3) Is the construction 'if I was' ever grammatical rather than 'if I was'?

4) Shouldn't there be the modal 'would' before 'be' in bold? Why has it been left out?

Sorry to add yet another question on conditionals. I hope this will be the extent of my confusion: It seems odd that she would mention her depression if she does not want to discuss it. If this is conditional, is the conditional this part: if she does not want to discuss it, it seems odd = Conditional one The ' she would mention her depression' is neither subjunctive nor part of the conditional, is this correct?
