Hi

"If John hadn't responded in such an aggressive manner he....."

a) would never have a black eye

b) would never have had a black eye

Do you think that both A and B are correct?

a) refers to the future - it means that John doesn't have a black eye yet
b) refers to the past - it means that John has it already

So if John has it then the B) is correct and A) is incorrect. This is likely to be the case here.
As for the A) I am not sure if it possible but I don't exclude it.
