To me, "concerted" feels more planned and deliberate.
If my daughter and I make dinner, say I do the main course and she puts together a salad and a side dish, we could say the result was a joint effort.
On the other hand, if we jointly plan a dinner party, do the shopping, work together on a couple tricky dishes, set the table in a more fancy manner - that might be a "concerted
