What is the proper way to punctuate the following sentences, specifically as it relates to "including" and "except"?
A complete review of systems including constitutional symptoms, eyes, ears, nose, mouth, throat, cardiovascular, respiratory, gastroenterology, genitourinary, musculoskeletal, integumentary, neurology, psychiatric, endocrine, hematologic, oncologic, immunologic, lymphatic and allergic systems were reviewed. All were essentially negative except as follows. The patient gets frequent headaches. He has a history of dizziness. He has chronic constipation and nausea, numbness of the extremities. He has a history of asthma and abdominal pain. He also has arthritis.
