Commas in indirect speech

When the "that" of indirect speech governs two independent clauses joined by a conjunction, should a comma be used before the conjunction? For example,

John said that Mark is watching TV at home and Betty is still working at the office.

or

John said that Mark is watching TV at home, and Betty is still working at the office.

I sure would appreciate your response.

I wouldn't use a comma, but I may repeat the "that."

"Don't shout,I can hear you," Susan said to me.

In the above sentence, should the conjunction 'because' be used in the place of comma (,)?
