Commas and quotation marks for words that aren't quotes

Hello. Keep in mind this is American English. I know that commas and periods always go inside quotation marks when it is used as an actual quote; however, what if it is not a quote? For example, this is the introduction of a character for a play I am editing:

The "Voice," apparently a doctor at the East Sanitarium.

Does the comma go inside or outside "Voice"?
