Commas

So for the next since sentence, I need to add commas where needed, so this is what I added:

The doctor who could diagnose illnesses that puzzled his colleagues may have been the model for the great Sherlock Holmes.

The doctor, who could diagnose illnesses that puzzled his colleagues, may have been the model for the great Sherlock Holmes.

I am wondering if I need a comma after illnesses as well??
