Question

For the Japanese placement test the professor has asked me to fill out a questionnaire, is that necessary for the Spanish placement test as well? Should there be a comma or period?  Thanks!

It's two completely separate ideas. It should be fairly easy for you to see that because the first is a statement and the second is a question.

It's always a mistake to put complete sentences together with just commas between. If it weren't a mistake, we would have entire books with just one period — at the end of the book!
