Cleft sentences

I have a lot of trouble in cleft sentences. Here are some.

1. Who or that is the best choice for the blank in the following sentence?

It is John...........often tells me about you.

2. When or that is the best choice in the following sentence?

It was on November 5th .............I was born.

3. Can where be used as in the following sentence?

It was at Lan's shop where I bought this shirt.

4. Can which be used in cleft sentences as in the following sentence?

It was my dog which was hit in the accident.

5. Can prepositions be used before relative pronouns in cleft sentences as in the sentence?

It was Anna to whom he talked at the party last night.

6. Which relative pronouns can be used in the structure: It is/ was + focus + relative pronoun +......?

You are free to give your opinion but the answer with grammatical background (what book, where you get the answers) is better.

These are my choices:

1. "It is John who/that often tells me about you." -- both are acceptable to me.

2. "It was on November 5th that I was born."

3. "It was at Lan's shop that I bought this shirt."

4. "It was my dog which/that was hit in the accident." -- both are acceptable to me. I prefer "that".
