In rare incidences, I have trouble making choices bwtween putting the "to be" form of verb and "do/did" form of verb? Can you refer me to a thread/site that can help me or explain to me here?Sorry for presenting a question that might be rather broad.
Also, is it right to write "a particular form of verb" with no article in front of the word "verb"?

I tend to think the "to be" verb form to be something that represent the state of being and when verbs involve/note some sort of activity, I usually get them right but once in a while, a thing or two play tricks on me and get me dithering between choices like the second set below.
Where is he playing basketball? (This is in a "to be" verb form and is correct.)
