Choked my breath from my lungs ~as~ my heart was wrenched from my chest

Can I use "while" or "and" instead of "as" in the following sentence?
"What I saw choked my breath from my lungs as my heart was wrenched from my chest."

And please tell me if "my heart was wrenched from my chest" has a purely figurative meaning? Because "choked my breath from my lungs" doesn't sound so strong.
