Cheque book request letter

Dears,

I want to write a letter to our bank manger requesting for a new cheque book for our company account. Last month our cheque books was lost and we already informed the bank and they cancelled the lost cheque series. Our bank representative will carry this letter to bank and he have to collect new check book

Anyone can help me to write a requst letter with the above mentioned matters
