Checking the sentence?

Please look at the following sentence,

"When uncertain about what to do in a work-related situation, asks your supervisor"

[Q1] What words are omitted between "When" and "uncertain"
I think it is "it is ", not "you are". Am I right?
[Q2] Why did the writer write as "asks your supervisor",not "ask your supervisor"? I couldn't understand it.
Could you help me with understanding it?
