Changing to present tense

Thank you all for the kind postings.

I am into medical transcription and in one report in Physical Examination part, doctor starts sentences in present tense but later changes to past. So as per account rule, I tried to transcribing those by changing tenses until I encountered the sentence:

"I did do a bimanual exam."

Kindly give me the correct transformation for present tense in this context.

"I did do a bimanual exam."

This verb form suggests the writer wants to add emphasis.

eg I did not do an ABC process, or a DEF process. ( However,) I did do a bimanual exam.
