Change 0f designation & salary adjustment

Hi, good day! Can you help me compose a letter to our HR department containing the change of my designation from Administrative Assistant/ Receptionist to HR & Administrative Assistant/ Receptionist. I started working here 2 years ago, after six months our HR Assistant resigned and her duties were endorsed to me. The company didn't get replacement until after 9 mos but resigned again after 2 mos. We've got a new one which lasted only week. After 2 mos, again we've got a new one which lasted only for 3 mos. Everytime the assistant resigned, all works were endorsed to me. How can I make a letter regarding this? I think it is also right that I ask for salary adjustment for doing works which is not on my job descriptions? Please help me with this...
