Hello, I have to translate a Certificate of Experience form Arabic to English for a friend of mine. I have translated it but I need help from native speakers or skilled English teachers to correct me if there are any mistakes.

This is to certify that Ms. (her name)______________________________ worked as a teacher of Arabic for (school's name)_____________school in (city)__________for two academic years (2009-2010 and 2010-2011).During that period she taught Arabic at/for? various educational levels and we found her overall performance to be excellent (during her service).
We wish her all the best in her futute endeavours.
This docume was issued by ___________school in _________on 20th May 2011 upon the request of the teacher.

I have some questions:
1-Do I have to say ' worked as a teacher of the Arabic language' or is ' teacher of Arabic' correct?
2My friend taught Arabic to Elementary and preparatory students and in the original Certificate of Expereince (the Arabic on the levels she taught are not mentioned so is ' she taught Arabic at various educational levels ' correct or should it be ' she taught Arabic at various educational stages' ?
3-'We found her overall performance to be excellent during her service' should I use this form or should I say ' and her overall performance was excellent during her service' ?
Please help me out as I haven't translateled something like this before and I don't want to make mistake in an important document like this one.
