You have to elaborate it as :

1. H/O Myopia with blindness.

2. H/O Myopia c blindness.

3. History of Myopia with blindness.

Index:

H/O - History of

c - Stands for "With".

By the by, is it just myopia or degenerative myopia / Myopic degeneration? Please let us know in detail.

If you MUST abbreviate your phrase above, then use:

"HO Myopia c blindness" as the abbreviation for "history of myopia with blindness".

Myopia is not the same as degenerative Myopia.

Myopic degeneration is a term that is used, but I prefer degenerative Myopia.

Sorry Sri, but the FIRST definition of H/O is Hematology and Oncology.

The second definition of H/O is History of

The abbreviation HO stands for History of. The only and unique meaning of HO is History of.

Hello seniors, that is what the reason i asked question. what my opinion is there should be one abbreviation for a purpose. Two types of abbreviations for one item is something confusing. HO and H/O are two types of abbreviations available in two parts of the world.
