Capitalisation in compound surnames following a title

Hi there

I know that in the following example the first letter in a compound surname must be lower case: Mr Andrew van Graan.

What happens when you use just a title? Would it be Mr van Graan or Mr Van Graan? I saw the latter version on a spelling show on TV and they didn't see it as a mistake, so maybe there is some convention I'm not aware of.

I would appreciate any advice.
