I would be very thankful if you could take look at this sentence:
I request not to serve passport of my minor son, John Smith (ID XXXX), on Julie Smith (ID XXXX). without my, child’s father, Jimmy Smith (ID XXXX), permission in presence. Is it clear that Jimmy Smith is father of John Smith and he requests not to serve the pasport of his son on the mother without his permission in presence?

"serve a passport" should mean "issue for her". This is the situation: State authority should not issue a passport of a minor child to his mother because there is risk that she will leave the country with the child.
This is the sort of letter that is better composed by a lawyer than by a teacher. Before you even write, you need to be sure that you have the legal right to insist that your permission for the issue of a passport be obtained.
I, Jimmy Smith, request that you (eg. a government official) not to issue a passport in the name of my minor son, John Smith, to the child's mother, Julie Smith, without my explicit (eg. written) permission.
