Cannot find the appropriate words...

Hello!
I have a tricky sentence and I cannot figure out how to write it in English. So, my version is as follows:

Certificate drafted for submitting upon request.

Unfortunately, I do realize that it is impossible to understand this sentence. Meaning:
1. A person requests that authority A issues a certificate.
2. Authority A drafts the necessary certificate.
3. The aforementioned person can submit this certificate to another authority, for example authority B.

How can I rephrase it? This sentence is written as an explanation at the bottom of this certicicate.
