Can you help me

Is the following sentence correct? What do you understand by it?

"Livewell is a health care company which specializes in helping hospitals promote healthy living, raise disease awareness and grow their reputation using social media"

Could you please help me correct it if it is wrong?

Thanks

It's not wrong, but it is poorly worded. "Livewell is a health care company which specializes in helping hospitals educate their patients on wellness and healthy living and grow their reputation through the use of social media.
