Can we use 'then' when narrating a story in simple present?

Hi teachers,
When narrating a story in simple present, can we use 'then' as 'at a particular time in the past or future'?

I don't think so. Should I change it to 'now'? Is there a better one?

Hi, This is the context: The man looks at me carefully. Then he takes the cigarette out of his mount and smiles with his yellow teeth. On a second though, I guess I can use 'then' if it takes the meaning of 'next, after that'.

This is the context:
The man looks at me carefully. Then he takes the cigarette out of his mount and smiles with his yellow teeth.

On a second though, I guess I can use 'then' if it takes the meaning of 'next, after that'. Right?
How do I know when it means one or the other? is it just because of the context?
