Can somebody please correct these?

1) Is the word order in the first sentence correct?

Could you tell me when the next bus to Place A is?
when is the next bus to Place A? (I know this sounds perfect!)

2) Which is grammatical?
He has forgot to change the card / He has forgotten to change the card

3) What is the difference between "I only bought the table for him" and "I bought the table for
him only"? What is the difference between using single and double quotations?

4) Does the following pair make sense?
A: Hello!
B: Oh! You WEARING a cap (trying to indicate that I was able to recognize him because of that)
/ you GOT a cap! (trying to indicate that I was not able to recognize him because of that)

Usually I would ask:
Could you tell me when the next bus to Place A leaves (or arrives)?

When is the next bus to Place A?
You will hear this, but it is informal.
Better: When does the next bus leave for Place A?

2)
