Can somebody look into this letter

Can somebody check the letter bellow, I've already send it but it would be nice to see what can be fixed in the future.

Dear sir/madam,
I'm having difficulties filling the application form, some parts of it seem to be designed to be filled on paper. Especially the end of the first part of the application where one should fill if he is agreeing with the statement about the usage of the information.

Should I print the application and sign above yes/no or can I write something above them?
And if I have to print it and deliver it on paper where should I send the hard copy?

Something else, i think that you should somehow inform the first year students in UE-Varna because no one seems to know that the deadline is 16.10.2007, and actually few people know about the organization.

Are you sending this as an email? If so, I would suggest leaving out Dear Sir or Madam.

I am having some difficulty completing the on-line application form, as some of the parts seem to require a hard copy to be filled out. In particular, the end of the first part of the application asks the applicant to fill in [or sign?] the form to confirm acceptan

Thank you for the tips. Fortunately the person who will read this mail speaks English worse than me.

How should I begin official emails? Is "Dear Sir/Madam" a good phrase, and if it's not why?

Sir/Madam is to be used if you don't know the surname of the person to whom the letter is being sent. If you do know the surname then you should use that starting with Mr, Mrs, Ms or Miss. Good rule of thumb if your unsure of the marital status of a female then use Ms.
