Can or Could

Hey,

can anybody explain when to use can or could and give some examples?

I'm not very sure when to use it. Just for example in a simple question: Can/Could you do me a favor?

What is more common and why, what is the difference?

Could you do me a favour is better than can you do me a favour.

Other examples:

Could you, please help me? (has no connection with the past)

Could you pass the salt, please?

Could you, please lend me your book, pen?
