Can anyone tell me what this sentence means?

Urban planning may be regarded as a rational process of forethought involving an orderly sequence of action that ultimately leads to the realization of a specified goal or goals concerning urban elements.

The sentence means exactly what it says. All words have their ordinary dictionary meanings. An explanation would probably be little more than copying dictionary definitions or finding synonyms.

(By the way, to my mind this is an almost totally pointless statement that adds nothing to anyone's understanding of urban planning.)
