Can anyone help me on these questions please?

Hello everyone, I am going to take an English exam very soon, thus I have done some english mock papers. However, there are some questions which I got incorrect answers. So I would like to ask if anybody can explain to me please? (If the above sentences are incorrect too, please correct'em)

Here is one of the question (Error Identification):

Equipment (A) breakdowns in industry accounts (B) for annual losses (C) of billions (D).

D. billions

E. No error
