What does it mean in the context: He called it in but didn't picked it up. (said by the pharmacists for a patient and his medicine).

One more question. How does it work? The patient calls and orders his medicine at the pharmacy?

I don't know exactly, but I guess if it's difficult for you to walk, or if you live far from the pharmacy/chemist's etc, you phone them to check whether they have the medecine you need. If they don't, they tell you when you can come & fetch it (they'll order it).

Unless it was a refill, the two "he"s are different in this sentence.

He (the doctor) called it (the prescription) in, but he (the patient) didn't pick it up.

No, there are no two he. There is only one person. The patient called it in but he, the patient did not pick it up
