By the way I think this example can't be classified in any of our previous examples:

"Smile looking at him" meaning "simultanously smile and look at him (at the same time)". Is it OK to say without the "by"?

This one should have "while":

Smile while looking at him.

Also, possibly, but less often, without "while": Smile looking at him.

"by" is totally wrong.

The relationship of the -ing clause to the main clause is not always the same. Sometimes it's "while"; sometimes "by"; sometimes "in" or "thereby" or "because"; sometimes other words. It
