Business Letters

On my courses of translation, i started studying the topic Business Letters. i faced a trouble from the very begining: the problem with greeting and ending the letter. how should i begin a letter if i write to a company, to a man, unmarried or married woman? what's the best ending: best wishes or truly yours or yours faithfully? Please, help me, because I think without help i won't be able to understand it))

To whom it may concern; Dear Sir or Madam (you probably don't know if the woman is married or not, but I think this form works). Yours truly, Sincerely yours.

i've also seen two variants: faithfully yours/yours faithfully and sincerely yours/yours sincerely. i've heard that one of this variants from each pair is american and the other is british. who can tell me where american and where is the british one?
