The only word that could go in the blank and sound natural is "the":

"Who broke the leg (on the table, the horse statuette, etc.)?"

You could in theory say something like:

"Who broke his/her/a leg?"

But it would be obvious who had a broken leg, so this sentence is of theoretical interest only.

I'd say this is a good example of the use of singular "they" when you don't know the gender of the person referred to. The first response that came to my mind was "Who broke their leg?"
