Breach, offense?

Dear Teachers,

What terms would you use to express the fact of students’ not acting according to the rules of the school (e.g. they don’t pay attention or they are talking during lessons)? I was thinking about “breaches”, “offense”, but even with the thesaurus I don’t know what words I should use… Please, could you help me? Thank you. =)

Thanks; the context I think of is, e.g.

During the lessons, pupils usually behaved well, but at times there were some ……..(e)s.
or
Concerning behaviour, in this case, more serious ……….(e)s could be observed as well.
