Born five miles over the water

I've heard on the radio the noun phrase "somebody born five miles over the water".

Does the distance of the five miles include the stretch of the water?

By default, I would understand "five miles" to refer mainly or entirely to the stretch of water, i.e. the stretch is about five miles wide. However, it is not possible to be certain without further information.
