eg Tom's mother cooked him a great dinner, with chocolate ice-cream to boot.

The chocolate ice-cream was a good and unexpected addition to the meal.

Maybe how often one hears that in casual conversation differs regionally. It's pretty common in the south and southwest, but then those types of folksy expressions are more a part of ordinary conversation, in my experience, in those regions of the U.S.
