Better way or business way to say that report went wrong .

Here's what I am trying to deliver..

There seem to be an issue with the report that I generate every week. I am researching to see what cause the problem at the moment. Do you want to see the report now ? or wait until I find out what is really going on and generate the accurate one ?

I couldn't figured it out how to say this in a more professional way.. it sounds like a kid talking..

I'd give it a heading : Faulty report. There currently seems to be an issue with the report I generate each week. I am looking into the cause of the problem, at the moment.
