Best way to write a letter to a bank, plz help !! asap

The bank has asked to give a letter saying that the son is allowing the mother to collect the gold jewelry which had been kept as security(guarantee) by the bank for a loan. the loan had been issued before father died. the loan has been paid now. So kindly help me which is the best way to write the letter. Is the following letter correct ?

To:
The Bank Manager,
National bank of -,
------------ Branch.

Dear Sir,
I, Mr. ------------ authorize Mrs.-------------- w/o Mr.---------------- to collect the gold jewelry kept as guarantee/ security at your end against the loan, issued on 00/00/2003. The loan has been paid off, the date of last installment being 00/00/2005.

I'm a tad confused. If the loan was to you, why mention your late father? If the loan was to your father, why do you need give permission to your mother?

In any case, the grammar seems ok.
