"Belated happy birthday" is better.

This is because you should keep the "happy birthday" bit together. And the "happy birthday" is what is belated. If you say "Happy belated birthday", it means the birthday is late (impossible!).

I myself say it a 'Belated happy birthday. But I have seen many people saying it as 'Happy belated birthday'..

Grammatically speaking, it should be belated happy birthday. Because as my friend said, the happy birthday i belated no the birthday itself.

Ex. Accept my belated happy birthday.

But as you mentioned, the other form is more common and is in widspread use.

But I would recommend you to stick to the one with correct grammar.
