Being challenged/all-cotton

Hi

The standard sewn mattress and box spring are being challenged by water beds and Japanese-style futons. Although synthetic fibres and all-cotton mattresses are the norm today, 100 years ago great attention was paid to the type of feathers used in the construction of the mattress.

Could "challenge" mean here "squeeze out of the market"?

Does "all-cotton" mean they are made only of cotton?
