Begining a letter to my boss

i need my manager approval my annual leave. Cuold I start my writing such as:
Dear V,
I hope you have a good holidays.
I would like to take 6 days my annual leave. I have filled the Application For Leave. I would be grateful if you could approve my leave. I will be back to the office on the 28th of January 2013.

Dear V, I hope you have (use "have" if the holiday is in the future; use "had" if you mean Christmas and New Year's, which is already past) a good holiday s . I would like to take my six days leave, from XX January to 27 January. I have filled out the Application For Leave, and would be grateful if you could approve it.
