2. Do you say

do exercise regularly or do an exercise regularly?

After "before", which is a preposition, we shall use the gerund if we use a verb.
So we can say:

Why don't you take a shower before... sleeping?
... going to bed?

And, about the second question, I think you can say:

Exercise regularly.
Do exercises regularly.

It should be '... before going to sleep'.

'Before sleeping' is grammatically correct and possible here, and no doubt it is occasionally used, but it is not the usual utterance.
