Been approved for declaration

-For students who have applied and been approved for declaration/change of major, minor(s), study period, they should check carefully on 'My Academics' whether the records have been updated. If there are any discrepancies, they must contact the RES for clarification.

1. Could you help me correct my sentence? APPROVE should be a correct word here, but I don't know how to fit it into the structure; saying 'who have been permitted to have declaration/change of major' is verbose.

2. 'My Academics' is an item on a website for course enrolment; can we say 'check on ... whether...'?
