3. The letter has been sent. Present Perfect Form
4. The dog has died. Present Perfect Form
5. The dog has been dead for 20 minutes. Present Perfect Form

A. In #2, you can't say "The letter has sent.". It's wrong. So it must be in the passive form, and you should include "been" in your sentence.
B. Because in #5 we have an adjective, which is "dead", in the sentence. So the word "been" is mandatory. If you leave it out, your sentence will be wrong.
