Which one is correct?

The dosage should be adjusted according to the symptom of the patient.

The dosage should be adjusted depending on the symptom of the patient.

The dosage should be adjuste based on the symptom of the patient.

The first sentence sounds correct, but what about the other two sentences?

If there is anything wrong in those sentences, please tell me where and why.
