Assistance with sentence tenses please

Please tell me if the following are correct:

I could have passed the exam if I had studied harder.
If I could have studied I would have/had passed the exam.
I would have passed the exam If I had studied harder.

Which are correct and incorrect?

Thank you. For the second example, is both 'have and had' possible prior to the word passed?

Also, are there other ways of saying these sentences as in the past in terms of things that could have turned differently? What I am trying to say is that what ever I say with if and in the past is considered hypothetical?
